Digital engineering firm Persistent Systems has announced a generative artificial intelligence (AI)-powered population health management (PHM) Solution in collaboration with Microsoft. This solution is designed identify Social Determinants of Health (SDoH) to enhance patient care and predict the cost of care associated with clinical conditions. SDoH are the non-medical factors such as economic stability. 

The PHM Solution utilises predictive risk modeling, a crucial tool for identifying individuals requiring significant medical assistance. By leveraging Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights, Persistent's solution analyses Electronic Health Records (EHR) data to identify SDoH. The company claims that this solution can  accurately extract up to 80% of key patient information from unstructured clinical notes within an EHR a marked improvement over existing PHM solutions, which heavily rely on third-party data sources or survey data.

It aims to enhance patient outcomes through more timely and intelligent care recommendations, offering higher accuracy, improved scale, and increased compliance compared to current natural language processing-based approaches.

Persistent's PHM Solution also assists physicians and caregivers in recommending personalised interventions through community-based programs and care management teams. The objective is to prevent emergency department visits, avoidable readmissions, and extended hospital stays. The solution has demonstrated a reduction of up to 60% in 30-day readmissions by continuously monitoring correlations and outcomes to ensure quality patient engagement.
